How to change the address on your npi data registry

To work in the healthcare industry or run a healthcare organization, you need a National Provider Identifier (NPI) number. The National Plan and Provider Enumeration System assigns an NPI to all health care professionals.

The National Provider Identifier (NPPES Registry), which can be found at https://nppes.cms.hhs.gov, can be used to look up your NPI and associated information.

All information produced by the NPI Registry is provided in accordance with the National Plan and Provider Enumeration System (NPPES) Data Dissemination Notice.

NPI is a 10-digit permanent number assigned to you for your entire medical career. Once the initial application is enumerated and a National Provider Identifier (NPI) is assigned, there are two options to change/update the information on an NPI record.

Making Changes Online

To make an online change, go to https://nppes.cms.hhs.gov and follow the steps provided below:

NOTE: Changes to Date of Birth, Social Security Number, Reactivation of an NPI, and Deactivation of an NPI cannot be completed online and require a Paper Application/Update Form.

How to change the address
  • On the Home Page of the NPPES website, enter your I&A User ID and password.
  • Select the “Pencil” ICON to Edit the desired NPI application.
  • Access the page containing the information to be revised by clicking on the ‘Next button at the bottom of each page or by selecting the correct page from the left-hand navigation bar.
  • Please update your information on that page.
  • Please submit it when you’ve filled in all of the information.

Making Changes with a Paper Application/Update Form

Updates can be requested by emailing the NPI Enumerator and requesting one via mail, or by sending a Paper Application/Update Form, which can be accessed at https://nppes.cms.hhs.gov.
